Description
Nosofedril 30mg/4mg Tablets contain Pioglitazone and Glimepiride, a fixed-dose combination of two oral antidiabetic agents.
Pioglitazone belongs to the thiazolidinedione class and improves insulin sensitivity, helping the body respond more effectively to insulin.
Glimepiride is a sulfonylurea that stimulates insulin release from pancreatic beta cells, helping reduce elevated blood glucose levels.
The combination provides complementary antihyperglycemic effects and may be prescribed when treatment with both medicines is appropriate.
Nosofedril 30mg/4mg contains a higher dose of Glimepiride than Nosofedril 30mg/2mg. The choice of strength depends on the patient’s existing diabetes treatment, glycemic response, tolerability and risk of hypoglycemia.
The medicine is intended for use alongside diet, exercise and regular monitoring of blood glucose levels.

Contraindications & Precautions
Heart failure: Pioglitazone can cause fluid retention and worsen congestive heart failure. Initiation is contraindicated in patients with established NYHA Class III or IV heart failure and is not recommended for patients with symptomatic heart failure.
Hypoglycemia: Glimepiride may cause severe low blood glucose, particularly in older adults, patients with kidney impairment or those who miss meals.
Bladder cancer: Avoid use in patients with active bladder cancer and carefully assess treatment in those with a previous history.
Liver function: Obtain liver tests before beginning therapy.
Drug interactions: Strong CYP2C8 inhibitors such as gemfibrozil can substantially increase Pioglitazone exposure. The 30mg fixed-dose combination is generally unsuitable when a maximum Pioglitazone dose of 15mg is required.
Pregnancy and breastfeeding: Require individual medical assessment; this combination is not a routine treatment choice during pregnancy.
